(a) I agree that I will abide by all FFPS rules and policies as published in FFPS 'Parent Info' (on-line, paper form, or phone-in) and FFPS 'Parent Manual' (provided by the coach) and any consequences for violation of such;
(b) FFPS may utilize soccer photographs, including those on FFPS 'Team Websites', of the named individual(s) and I hereby waive all rights to the photographs and compensation
(c) I understand that FFPS will assemble teams based on a balancing system by ability primarily, and then grade, age, height, weight, school and friends are secondary;
(d) I understand that if my child’s coach does not contact me by the FFPS published latest date to obtain my child’s Team Roster, I am obligated to contact FFPS Hotline at 1-800-828-PLAY (7529) or go to www.ffps.org 'My Account' to locate and contact my child's coach;
(e) I understand that my child(ren)’s date of birth(s) must be accurate and intentional misrepresentation could result in removal of my child(ren) without a refund;
(f) I understand that I and all my family members must behave in an appropriate manner in all communications with any FFPS appointed volunteers or FFPS staff;
(g) I understand that I and my family members are expected to provide positive behavior at all FFPS functions and are not permitted to give game strategy or instructions at games to players or confront a coach or another parent at any FFPS function so as to disrupt it in any way. Violations of FFPS policies or inappropriate conduct may result in suspension or removal of the adult and / or player;
(h) I understand practices are held once a week usually during the week in the evening or on Sundays, and for approximately one hour at area schools and parks. I understand that I must follow all behavior policies at games and practices. I will always be a positive fan by using positive parenting at all games (e.g., go…., good…., great…., nice…..;
(i) I understand that to ensure a positive experience 'for all the kids' I agree to the following:
(i) No rude, inappropriate, foul language, violent, or disrespectful behavior towards anyone in FFPS.
(ii) No coach strategy or 'How to Play' instructions to the players allowed, discussion of 'concerns' with any coach or parent during the game (only to your own coach), or yelling loudly.
(iii) No dogs, tents or canopies (only small hand held umbrellas allowed), bikes, standing behind goal, illegal parking or driving, and help pick up any trash to ensure a safe clean event.
(iv) No smoking, drugs, or alcohol allowed.
(v) Following any further FFPS Policies/Procedures provided by your coach.
(vi) Shin pads and proper soccer attire must be worn at all times when playing. No earrings, objects in the hair, bracelets, necklaces, or any jewelry or watches are allowed.
(viii) Always use positive comments and encourage the players. Must say positive words like 'Go..., Good..., Great..., Nice...' before you say anything else.
(ix) To not exhibit derogatory or disruptive behavior. If it occurs from others I will only attempt to stop it by walking away with no further discussions or actions. If that fails to go home and contact FFPS immediately at 800-828-7529 x911.
Any violation of FFPS Policies or conduct of an inappropriate manner mentioned above toward any staff, Head Volunteer, Coach, player, parent/guardian, or visitor, FFPS may require that person be removed temporarily or permanently and if permanently with or without a refund
(j) I understand that I may request a friend by following FFPS Procedures and Policies located at www.ffps.org 'parent Info' and that my request is not guaranteed and can be denied at anytime;
(k) FFPS VOUCHER AND REFUND POLICIES:

- If paid in full, and season canceled or I chose not to participate before the first game, for named individual(s) __|CHILD_NAME|__ FFPS will provide a 100% Voucher to any future FFPS Season or location (if FFPS had a season where not all games were played (ie: weather) FFPS will determine if pro-rated amount applies).
- If I had A '2 payment plan', and season canceled or I chose not to participate after the first game up to the 4th game, for named individual(s) __|CHILD_NAME|__, FFPS will provide a 50% Voucher to any future FFPS Season or location (if FFPS had a season where not all games were played (ie: weather) FFPS will determine if pro-rated amount applies)
- After the 4th game no Voucher or Refunds available
- SCHOLARSHIPS: No credit or refund for those who already received a scholarship
REFUND EXCEPTIONS:
- If moved away more than 30 miles from any FFPS location and my child misses Up To 50% Of Games.

* Wait List:
If teams are full when you register, you will be given a choice to either be put on the "Wait List" or not be put on the Wait List and 100% Refund or Voucher returned if do not wish to continue on the Wait List.
RULES:
- If you choose to be put on the "Wait List" you must make a full payment, either with a Credit Card or with a Voucher to guarantee your placement if an opening should become available.
- If you chose to not be on the "Wait List", then no payment is needed and no reservation will be set up.
- If you choose to be on the "Wait List" but change later, your Voucher will be returned, or, your payment 100% refunded.
- Openings on teams is guaranteed if register before "Guaranteed Placement": date.

GENERAL REFUND POLICIES:
- There are no refunds for any other reasons including dissatisfied team assignment, friend request denied, age of teams (too old Or too young), practice or game conflicts, Or any delays in communicating with your Coach, Or if my team has no coach yet. If my coach does contact me by the 'latest coach call date' (see season schedule) I am obligated and responsible to locate contact information and contact my coach call the FFPS Hotline at 281-347-5425 (KICK) x913 or x911 to locate my coach for my child. I understand FFPS is a volunteer program locally so, I have a responsibility to help the team in any way I can, even if my team has no coach yet.
- When an area is under a Presidential Disaster Declaration or if the Presidential Disaster Declaration was recent, FFPS may adjust its Refund Policy at any time to ensure its financial viability. NOTE: No refunds if a Financial Scholarship was provided.
PRESIDENTIAL DISASTER DECLARATION:
When There Is a Presidential Disaster Declaration, or Presidential Declared National Emergency, or State Declared Declaration, or State Declared Emergency: "... refund requests will be accepted but Not considered until FFPS's financial viability Is determined". FFPS may adjust its Voucher/Refund Policy at that time to ensure its financial viability. We would appreciate your understanding when this occurs.

Can I Transfer my Voucher?
Yes, as long as it someone in the same household (same address), and with at least one Parent / Guardian the same.
No Voucher or refund for those who already received a scholarship
(l) If my team does not obtain a Business Sponsor for $420, I agree to pay a sponsor fee of $45 per named individual player(s) due Picture Day (mid-season);
(i) I authorize FFPS to charge my credit card $45 per named individual player(s) If my team does not indicate they have obtained a Business Sponsor For $420 up To 5 days prior To Picture Day (mid-season).
(ii) If I do not make payment to FFPS as agreed for initial registration fee or subsequent registration payments or if my team does not obtain a Business Sponsor for $420, I agree to pay a sponsor fee of $45 per named individual player(s) due Picture Day (mid-season) and if I do not pay such $45 fee by Picture Day, I agree that FFPS has the right to:
(a) not allow my player to continue to participate in any FFPS team for the remainder of the season,
(b) deny my player's trophy,
(c) be accepted for a financial scholarship,
(d) not allow my player or other family members to participate in any future seasons in FFPS.
(iii) I understand the sponsor fee does not include pictures. Pictures are taken mid-season and are optional. Packages range from $21 to $40.
(m) I understand that with regards to my personal information:
(i) My personal information provided to FFPS will never be given to any 3rd party, such as a business, vendor, or person and can only be used by FFPS to inform me of FFPS approved functions unless you consent otherwise. FFPS will never sell or profit from your information. You may request removal of any personal information to FFPS Director of Administration at 1-800-828-PLAY (7529) or kathlyn@ffps.org at anytime;
(ii) FFPS collects personal information at sign up and in Volunteer Forms for FFPS internal purposes, including contact purposes, team balancing and formation. FFPS may provide all or part, or none of the following information to all parent/guardians of their team as the 'Parent Copy' to include Coach and Team Manager Name (first and last name) and home, work, and cell phone numbers and email address, and, all Parent/Guardians Names (first and last name) and their relationship to the player, work and cell phone numbers, email address, and, Player name (first and last name), home phone number, years played, years coached, and gender. FFPS may provide all, or part, or none of the following information to the Coach only as the 'Coach Copy' to include Coach and Team Manager Name (first and last name) and home, work, and cell phone numbers, email address, and, all Parent/Guardians Names (first and last name) and their relationship to the player, street address, city, zip code, work and cell phone numbers, email address, and, emergency contact and phone number, volunteer selections, and, player name (first and last name), date of birth, home phone number, school, and gender, years played, years coached, and, any special need/consideration information. All parents, Coaches and Team Managers may only use that information for FFPS purposes. If anyone is caught misusing this information or using for any purpose other than FFPS purposes, they will be subject to FFPS discipline, including the possibility of total separation from FFPS.
(n) I understand that FFPS secures and protects all information transmitted over the Internet through the use of encryption. Transfer of secure, personal information is always encrypted. All transactions that contain personal information use SSL encryption with 'Secure Web Server Certificates'. FFPS only requests or displays parent information through forms located on our secure web servers;
(o) I understand that FFPS’s continued efforts to protect the children requires that FFPS perform a background check prior to each season of play on any coach, staff, vendors, or those FFPS deems it is in the interest of the children's safety. If I am denied a position in FFPS due to this background check, FFPS will notify me in writing in 10 days and I may request a copy of my background check. I may appeal the decision within 30 days;
(p) I understand that FFPS uses a 3rd party vendor when a background check is required and since it is a national search it is required that FFPS obtain and provide a Social Security Number (SSN) to ensure the identity is correct as duplicate names can arise with such a large population being searched. In addition to the above security protection (see 'Security Policy' above), FFPS places another level of security on SSN and FFPS will never show the SSN to anyone, in any format, including the person who provided it. The only FFPS persons allowed to access the SSN, if needed, are the FFPS Director of Administration and FFPS Background Screen Administrator who is appointed and monitored by the FFPS Executive Director.
(r) FFPS will discard all personal information and SSN at the child's 20th birthday at the latest and will notify you if this will change due to any program FFPS might have at that time that it would be needed in, like an adult soccer league or alumni program. I understand I may have my information discarded at anytime by contacting the FFPS Director of Administration at 1-800-828-PLAY (7529) or kathlyn@ffps.org;
(s) I understand that FFPS will communicate upcoming events and sign up reminders via email, phone, and mail. FFPS will send at times, my User Id. No. and Password to remind me to use them so to ensure all past information is available to assist in balancing teams. FFPS therefore recommends that I use a password that is unique, not one I might use for my bank, etc. If I would like to opt out of emails, mail, or phone communications I can do so in those communications as there is an opt out selection or link, or contact FFPS Director of Administration at 1-800-828-PLAY (7529) or kathlyn@ffps.org.
(t) I understand that FFPS balances teams using criteria established by FFPS and once my child is placed on a team by FFPS that I may request to move to another team. I understand that I may only make this request at www.ffps.org 'My Account', 'Swap Players'. I understand that it may be difficult to move to another team as the teams may be full and it may violate the FFPS Balance Team rules. All attempts by the FFPS's online 'Swap Players' are final. FFPS has the right to deny or change any request at anytime.
(u) I authorize FFPS to charge my credit card for any additional amounts that I may owe FFPS during the season.
(v) I authorize FFPS to send confirmation information to any family members that I provide email address and/or mailing address.
(w) I understand that FFPS Coaches or Assistant Coaches or Team Managers or any FFPS Volunteer so designated as such by the FFPS team or by FFPS are not permitted to use social media as a representative of FFPS.
(x) I understand that FFPS does not discriminate on the basis of religion in any of its activities and coaches, assistant coaches, or anyone acting as the coach are not authorized to conduct religious activities during and FFPS events.
(y) I understand that $100,000 accidental insurance is included with registration and has a $100 deductible. It is secondary coverage so it pays after your coverage unless you do not have insurance. For claims or information call Jack Hendrie at 800-828-PLAY(7529) ext 911 within 48 hours of an accident to obtain forms and for verification of the incident. All claims must be sent to FFPS within one year.
(z) I understand that FFPS registration fees are expected to be paid in full and that any discounts publicized for the 3rd, 4th, 5th child or more, cannot be combined with a league based promotion or discount.
(aa) I accept that FFPS uses parent volunteers to Coach, Asst. Coach, Help Coach, Team Manager, Time Keeping, Substitution, Flyers, Field Lining, Registration, Sponsor Coor., Parade Coor., and other duties locally needed and that I may be asked to volunteer. Volunteering is not required but fun, rewarding, easy to learn and do, and greatly appreciated. FFPS provides extensive support and training for the volunteers.
(aaa) I understand that if I am asked and agree to Volunteer to Coach that I will:
(i) Follow all the training provided and follow and enforce all FFPS Policies and Procedures provided in the FFPS Manuals, Rule Books, and Packets.
(ii) If I decide to use the FFPS Website, and if I allow pictures to be shared that I will only approve pictures to be viewed that are wholesome and that are of a family type pictures meant for young children to view.
(iii) Be respectful of others.
(iv) Follow 'FFPS Substitution System' to ensure fairness.
(v) Not allow myself or others to exhibit derogatory or disruptive behavior at any FFPS event and if it occurs that I will either walk away with no further discussions or actions. If that fails, to end the event and ask everyone to go home and to contact FFPS immediately at 800-828-7529 x911.
(vi) To ensure all players wear safe and appropriate soccer attire and shin pads while playing at all times. To ensure that no earrings, objects in the hair, bracelets, necklaces, or any jewelry or watches are allowed.
(vii) Not write down or say the score or standings at anytime. The players may do so.
(viii) Only cancel games if due to poor weather or less than 8 players between both teams and ensure that all games are played.
(ix) Only allow players to play in games that are on the FFPS Official Team Roster. The only exception is if missing players. Any players added must be approved by the opposing coach. For exceptions procedures see the 'FFPS Coach Policies'.
(x) Ensure the 'Medical Authorization' forms are received on every player and kept in your possession or if not attending a person you appoint at every FFPS event.
(xi) Review and follow the 'FFPS Child Abuse Section' contained in the FFPS Packet.
(xii) Obtain all training required.
(xiii) To ensure that a Team Sponsor (Business ($420 per team) or Parent ($45 per player) is obtained and ensure:
(a) I authorize FFPS to charge my credit card $45 per named individual player(s) if my team does not indicate they have obtained a Business Sponsor for $420 up to 5 days prior to Picture Day (mid-season).
(b) If my team does not obtain a Business Sponsor for $45, I agree to pay a sponsor fee of $45 per named individual player(s) due Picture Day (mid-season) and if I do not pay such $45 fee by Picture Day, I agree that FFPS has the right to:
(i) not allow my player to continue to participate in any FFPS team for the remainder of the season,
(ii) deny my player's trophy,
(iii) be accepted for a financial scholarship,
(iv) not allow my player or other family members to participate in any future seasons in FFPS.
(xiv) To ensure all player's parents or guardians are contacted as soon as Rosters are available to you from FFPS or at the latest by the FFPS published date.
(xv) Always use positive comments and encourage the players. Must say positive words like Go..., Good..., Great..., Nice... before you say anything else.
(xvi) Not use 'How to Play' instructions at games. Only correct players in a positive way, like 'kick the ball, not the shins'. No derogatory or negative language is allowed.
(xvii) Submit to a criminal background check and provide the information needed when requested to do so timely and accurately. See above sections for more information on background checks.
(xviii) I understand that if I am asked and agree to Volunteer to be a Team Manager that I will follow all the duties outlined in the 'FFPS Team Manager Duties' Packet. I will see my Coach for this information.
